Designated Coordinator (RN or LSW)

Position Summary

The Designated Coordinator (DC) is responsible for the overall management, quality assurance, and regulatory compliance of 245D Home and Community-Based Services (HCBS). This role ensures that services are delivered in accordance with Minnesota Statutes, Chapter 245D; the Positive Support Rule (MN Rule 9544); and DHS expectations. The DC oversees and ensures service planning, staff competency, incident response, and collaboration with individuals served, their support teams, and agency personnel.

Qualifications:

Bachelor’s degree in a field related to human services, education, or health plus one year of full-time direct care experience, or
Associate degree in a related field plus two years of full-time direct care experience, or
Diploma or certificate from an accredited postsecondary institution in a related field plus three years of full-time direct care experience, or
Minimum of 50 hours of education and training related to human services plus four years of full-time direct care experience, AND Have a minimum of three years of supervisory-level experience in a program that provides care or education to vulnerable adults or children.

Essential Duties and Responsibilities

Develop, write, implement, and oversee CSSP Addendums and related service planning documents that reflect the unique needs and preferences of individuals served

Review service documentation for accuracy, timeliness, and alignment with 245D licensing guidelines

Conduct routine file audits and ensure timely completion of required forms, annual documents, and authorizations

Maintain current knowledge of 245D, Rule 9544, the Vulnerable Adult Act, and all related regulatory requirements

Implement corrective actions from internal reviews, licensing visits, and DHS audits, and ensure all DHS-ordered corrective actions and licensing requirements are fully met to maintain continuous compliance

Provide staff training, supervision, ongoing competency evaluations and performance feedback

Ensure all agency staff maintain required training compliance and competency under 245D

Support service delivery and designated coordinator duties during gaps, call-offs, or emergencies

Lead ongoing program improvement initiatives and quality assurance efforts

Lead and participate in support team meetings, person-centered planning sessions, and internal office meetings

Collaborate with case managers, legal guardians, and individuals served to ensure alignment of services with personal outcomes/goals

Maintain and monitor service delivery and documents to ensure services are person-centered, respectful, and aligned with positive support strategies

Evaluate individual and support team satisfaction and use data to improve services

Respond to and manage emergency or crisis situations in real time

Lead and document internal investigations related to incidents, injury, or suspected maltreatment

Ensure all incident reports and required follow-up actions are completed accurately and timely

Core Competencies and other requirements

Deep knowledge of Minnesota Statutes Chapter 245D and Rule 9544 and complete required annual training

Strong documentation oversight skills

Professional communication and collaboration skills with interdisciplinary teams

Familiarity with electronic documentation systems and compliance tracking tools

Must have reliable transportation and be willing to travel to service sites

Pay: $30.00 - $35.00 per hour

Expected hours: 40.0 per week
