Experteer Overview In this role, you develop and implement person-centered treatment and discharge plans for adults with serious mental illness, collaborating with a multidisciplinary team and community partners. You ensure timely assessments, discharge readiness, and accurate documentation while aligning with forensic and mental health regulations. You engage families and CSB liaisons to support recovery and coordinate education and planning across settings. Compensation / Benefits • Develop and implement person-centered treatment and discharge plans with consumers and the treatment team • Complete SW assessments within 7 days of admission and assess discharge needs with team input • Complete clinical readiness for discharge ratings and issue EBL notifications within 24 hours of addition • Provide PSR interventions (roughly 1–2 hours weekly) • Ensure discharge plans reflect knowledge of forensic service delivery and mental health law • Collaborate with CSB liaisons and notify CSB staff of treatment planning conferences • Participate in interagency SW-Liaison meetings and Regional Census Meetings • Engage families and others with consumer consent in assessment, treatment, and discharge planning; provide education to families • Conduct family meetings with the team and ensure robust clinical documentation per Joint Commission, CMS, and SW standards Tasks • BSW required (CSWE-accredited program) • Professional work experience with adults with serious mental illness • Discharge planning experience (Additional Considerations) • Veteran or veteran spouse (Additional Considerations) • Bilingual (Additional Considerations) Key requirements • retirement with cash match • paid time off; sick time; family/personal time; community service time • health, vision, and dental insurance • life insurance
