Delivering H.O.P.E. Human Services Inc., has been serving the RVA community for fourteen years. Our response team includes fully Licensed Mental Health Professionals (LMHP). Licensed Mental Health Professionals that are residents in Social Work and Counseling, and Quality Mental Health Professionals (Adult and Children). Role Description

Services are primarily to be provided via face-to-face involvement with the client and other family members primarily in the clients home and in various community settings for Mental Health Skill Building Services and/or Intensive In-Home Services but services may also occur in various community settings for clients. The Qualified Mental Health Professiona (QMHP) is required to assist in the development and implementation of the person-center, recovery-based treatment plan. The activities associated with the primary responsibility of this QMHP are:

• Ongoing assessment of treatment needs.

• Assist in the development and ongoing monitoring and implementation of Individual Service Plan

• Work with clients to foster the development of new improved coping, problem solving, and communication skills.

• Work with clients to obtain the necessary independent living skills so they will be able to adaptively cope with life’s challenges.

• Maintain continuous contact with the identified consumer and other key stakeholders (with signed authorization to release information) in planning and service delivery. These stakeholders might include but are not limited to social workers, psychiatrists, juvenile court officials and police, teachers and other human service professionals

• Educate family members regarding existing community services, and act as an advocate in connecting family members to these services

• Respond to family crisis on a 24 hours, seven day a week basis

• Maintain up-to-date client records, and prepare required/necessary reports in a timely and accurate manner

• Link clients with all neceasary resources in order to maintain a continuum of care.

• Establish a good working relationship with other public and private agencies/services and interface collaboratively with them


Qualifications


Academic/Work Experience:

• Bachelor’s or Master’s degree preferably in the Human Services field.

• Candidate must be registered as a QMHP or QMHP-T with the Virginia Board of Counseling.

• 1-3 years of experience of directly working with individuals with Mental Health/Behavioral Health challenges are prefered.



Training:

• Staff must be trained certified in: Cardio Pulmonary Resuscitation (CPR), First Aid, and Therapeutic Options of Virginia (TOVA).

Additional Requirements:

• Employee must be certified as being tuberculosis free

• Employee must clear criminal background and CPS registry checks
