Staff Social Worker- LTC

Located in Queens, NY

**SALARY- $60K range (depending on experience)**

Job Description:

• Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living
• Provide information to the resident, and/or family member, on residents' rights and responsibilities

Qualifications:

• Must have BSW or MSW
• Must have experience as a Staff Social Worker
• Must have long-term care/nursing home experience
• Must have excellent outreach, assessment, written and verbal communication skills.
• Must be able to work effectively as a team.
