VNA of Albany is seeking a dedicated and passionate Community Liaison to join our team. As a Community Liaison, you will play a vital role in connecting our organization with the community, promoting our services, and building strong relationships. This is an exciting opportunity to make a positive impact in the community and contribute to the success of our organization.

Duties:

  • Provides clinical education to physician practices, assisted living facilities, and nursing homes in regards to the services provided by VNA to promote better understanding of patient benefits of home care and grow referral base.
  • Establishes relationships with physician groups, assisted living facilities, hospitals and nursing homes and acts as a clinical liaison.
  • Trouble shoots potential communication gaps and problem solves as necessary with physicians and community settings to ensure smooth and effective communication and case management.
  • Reviews active cases with clinical staff in physician practices and community settings to enhance communication and case management. Seeks clinical updates from and provides guidance and communication link to case managers.
  • Collaborates with CMs and physician practices in establishing clinical protocols and works closely with specific physician practices to implement it.
  • Establishes relationships with nursing homes and hospitals to educate in regards to VNA services and promote the transition of patients to VNA at discharge. Educates clinical staff in regards to home care services to be provided, assists with Intake coordination and preparation for discharge, and directs VNA clinical staff as necessary to promote effective transition to home.
  • Educates physician practices in the value of home care in keeping patients home and reducing hospitalization. Collaborates with physician groups in developing protocols and communication strategies to reduce avoidable admissions. Collaborates with CMs in implementing and educating clinical staff in protocols.
  • Participates in the re-hospitalization PI team.
  • Identify opportunities to conduct community health clinics with referral sources.
  • Develops and maintains appropriate reports.
  • Performs other related duties as assigned.

Requirements:
- Must possess current license as a Licensed Practical Nurse in New York State.

- Strong networking and relationship-building skills

- Excellent communication skills, both written and verbal
- Preference will be given to those who have had, in addition to public health nursing, preparation and, or other specialties in an accredited hospital or who have had experience with discharge planning, liaison, or coordination activities with a health institution, or sales experience in health care.

Salary:

$25.00 - $30.00 Hourly

Hours:

Monday - Friday

8:00 AM - 4:00 PM
