Experteer Overview In this role you will coordinate safe, timely transitions for residents as they move to the next level of care or home. You work with clinical teams, residents, families, and providers to ensure smooth discharge planning and continuity of care. You’ll handle planning across care settings, documentation, and regulatory compliance, helping to optimize outcomes.
