Full time Social Worker

We are looking for an individual who can take on the role of Social Worker at our facility. This job includes but is not limited to,

• setting up care plan meetings and documenting
• working with other departments to coordinate best plan of action for each resident
• strong follow up, advocacy and "people" skills
• evaluting residents using the MDS assessment, completing all required care plan and progress note documentation in the EMR
• education and completion of advance directives
• working well with assisted living facilities and adult homes as well as home care agencies to insure a safe and timely discharge for each indivual resident, referring residents to appropriate state run programs.
