Job Details:Position Summary
The Supervisor of Social Work provides clinical and administrative leadership for the hospital Social Work team. This role oversees daily operations, staff performance, and patient throughput while ensuring high-quality, compliant, and patient-centered social work services across inpatient and outpatient settings.
Supervisory functions
- Maintains a consistent pattern of attendance. Reports to assigned area on time for scheduled shift.
- Assists with the preparation of periodic reviews and evaluations as directed.
- Acts as a representative of the hospital in inter-agency or intra-agency groups as directed.
- Evaluate performance of social work staff.
- Serves as a member of appropriate hospital committees as assigned.
- Provides field instruction for social work students assigned to the hospital.
- Assists in the ongoing development of an up-to-date comprehensive file of resources available to assist patients, their families, and community residents.
- Adheres to hospital/department dress code. Arrives to work appropriately dressed, well-groomed and neat. Demonstrates good personal habits.
- Utilizes principles of infection control and standard precaution in the performance of patient care activities.
- Conforms to policies governing patient abuse, patient rights, and confidentiality of information.
- Performs all related duties as required and unrelated duties as requested during emergencies.
- Share information related to educational opportunities for social work staff.
- Collaborate with social work staff, Public & External Affairs and other involved staff as appropriate to maintain community support groups.
Key Responsibilities
- Supervise and evaluate social work staff; provide leadership, education, and support
- Provide direct patient and family services, including bio-psycho-social assessments, counseling, and crisis intervention
- Coordinate discharge planning, including transfers to rehabilitation facilities, SNFs, adult homes, and transportation
- Ensure compliance with documentation, QA/QI, and regulatory standards
- Screen and refer cases involving abuse, neglect, domestic violence, and protective services
- Collaborate with interdisciplinary teams to support patient care and safe discharges
- Participate in hospital committees, community partnerships, and student field instruction
- Maintain culturally competent, patient-centered care practices
- Tasks delegated by Director of Care Management
Qualifications
- Master’s Degree in Social Work (MSW) required
- Current New York State Social Work license required (LCSW)
- Prior hospital experience preferred
- Supervisory experience strongly preferred
- Strong knowledge of discharge planning, care coordination, and regulatory requirements
- Proficiency with electronic medical records and standard office technology
