Oneida Health is currently seeking a full-time Care Coordinator to work in Medical Practice Network Setting to include Primary Care and our Women’s Health practice in Oneida, NY. This position will assist Women’s Health and OB patients by working collaboratively with the members of the clinical team to identify disparities that prevent them from achieving optimal health care. The Care Coordinator in the practice setting is responsible for working collaboratively with members of the clinical team and in partnership with patients, families, and physicians to promote timely access to needed care, comprehension and continuity of care, and the enhancement of the patient's well-being. The Care Coordinator will be responsible for helping patients find solutions to the barriers in their health care.

What You’ll be Great At:
- Completing initial psychosocial assessments for patients including mental health and DV screening.
- Providing resources and making referrals to agencies in the area for parenting support and/or baby supplies.
- Contacting patients who are non-compliant and trying to help them return to care.
- Assisting with insurance questions including information about newborn enrollment.
- Connecting patients with resources for primary and dental healthcare providers.
- Developing care plans and supporting patients and families in self-management, self-efficacy, and behavior change.
- Facilitating patient access to medical home providers, transportation to and from visits as necessary, and adoption processes where applicable.

Your Essential Qualifications:

- Bachelor’s degree in Social Work or related field (Required)
