Mental Health Counselor

-Maintain a caseload and provide regular case management services to these individuals

-Maintain resident records in compliance with agency and funding standards

-Develop and implement individual Service Plans and assist residents with achieving their goals geared towards self-sufficiency

-Monitor and ensure residents are linked to healthcare providers

-Provide counseling and crisis intervention as necessary

-Assist residents in securing and maintaining benefits

-Maintain a liaison with community services, including hospitals, substance abuse treatment programs, HRA, and mental health programs

-Facilitate groups and activities that promote health and independent living skills development

-Monitor and assist residents with rent payments and budgeting

-Conduct regular unit inspections

-Educate residents about approaches to mental health

-Attend ongoing training to enhance the services provided to the residents

-Other duties as assigned
