Position Summary:
The Social Worker MSW provides education guidance and support to patients and families coping with emotional physical or social challenges. This role includes psychosocial assessments care coordination discharge planning and collaboration with the healthcare team.

Requirements:

• LMSW license required
• BLS AHA only
• Minimum 1 year social work experience
• Experience in comprehensive care management preferred

Key Responsibilities

• Conduct and document psychosocial assessments
• Develop and implement treatment and care plans
• Provide patient and family education and support
• Assist with discharge planning and care coordination
• Provide crisis intervention and community resource referrals
• Collaborate with interdisciplinary healthcare teams
• Prepare professional reports and documentation
