WESTARM Home Health is a locally owned, patient-focused home health provider serving the Alle-Kiski region. We pride ourselves on compassionate care, strong clinical support, and efficient coordination between patients, clinicians, and referral partners.

We are currently seeking a Home Health Coordinator to support daily operations, manage referrals, and ensure smooth communication across our home health team. This position is based out of our Corporate office in Lower Burrell.

Position Summary

The Home Health Intake Coordinator plays a key administrative and coordination role in home health operations. This position is responsible for managing referrals, supporting clinicians, communicating with patients and facilities, and ensuring documentation and scheduling are handled accurately and efficiently.

This role is ideal for someone who is organized, detail-oriented, comfortable with fast-paced healthcare environments, and confident communicating with multiple stakeholders.

Key Responsibilities

  • Receive and process home health referrals from hospitals, physicians, and facilities
  • Coordinate patient start-of-care and ongoing visits with clinical staff
  • Communicate with patients and families regarding scheduling and services
  • Maintain accurate patient records within the EMR system (Careficient)
  • Verify and track required documentation (orders, certifications, authorizations, etc.)
  • Serve as a primary point of contact between clinicians, referral sources, and internal staff
  • Assist with census tracking and daily operational workflow
  • Support compliance with Medicare and payer requirements
  • Provide general administrative support to home health leadership as needed

Qualifications

  • Prior experience in healthcare administration, home health, or medical office coordination preferred
  • Strong organizational and time-management skills
  • Excellent written and verbal communication skills
  • Comfortable working with EMR systems
  • Ability to multitask and prioritize in a deadline-driven environment
  • Knowledge of home health workflows, Medicare requirements, and HIPAA rules and regulations is a plus

Pay: $20.00 - $25.00 per hour
