Social Worker Qualifications

• Previous experience in a long-term care facility required in consulting

• Oversee facility social worker

• BS in Social Work

• A love for people, especially the elderly

• Excellent communication skills

Social Worker Responsibilities

• Counseling residents and families.

• Coordinating any necessary psyche services and behavior meetings.

• Discharge planning, participating in care conferences and communicating effectively with the nursing team.

• Keep accurate records for each resident you work with, including MDS logs, and report any changes in a resident’s condition to the nursing department immediately.
