Job Title: Social Worker - Acute Care (MSW)

Location: Simi Valley, California

Contract Duration: 13 weeks
Shift: 8 hr days (start between 8:00 – 9:00 AM), 30-minute lunch
Hours/week: 40hrs

Job Description:
We are seeking a compassionate and experienced Social Worker (MSW) to join our acute care team. In this role, you will provide support, counseling, and assistance to patients and their families, helping them navigate the challenges associated with hospitalization. You will coordinate transition plans, assist with post-discharge needs, and collaborate with nursing staff, outside agencies, and family members to ensure a smooth continuum of care.

Responsibilities:

  • Provide counseling and support to patients and families regarding non-medical issues related to hospitalization
  • Develop and implement transition and discharge plans in collaboration with care teams
  • Serve as a resource and advocate for patients and families during hospitalization
  • Coordinate with nursing staff, outside agencies, and relatives to facilitate patient care and discharge
  • Address routine social work issues and provide guidance to patients and families

Qualifications:

  • Master’s Degree in Social Work (MSW) – Required
  • Acute care experience: Minimum 1 year required; 2+ years preferred
  • Active California license
  • Strong knowledge of social work principles and best practices in acute care
  • Excellent communication, problem-solving, and counseling skills

Job Type: Contract

Pay: $2,520.00 - $2,560.00 per week
