JOB SUMMARY

Recommend and assist access to medical, educational, social, vocational, housing, and more to support program goals. Initiate referrals to next level care, coordinate with physical health providers for whole-person care, and advocate in the best interest of clients.

RESPONSIBILITIES

Reasonable accommodation may be made for individuals with disabilities to perform essential job functions.

    • Conduct weekly housing inspection, follow reporting requirements, and work with clients to remediate issues.
    • Provide case consultation, crisis intervention, discharge placement, and targeted case management services to assist clients in meeting basic needs and housing needs.
    • Complete accurate and timely documentation as contractually required and meet minimum requirements for direct billable client services.
    • Locate housing opportunities to meet short-term, transitional, and permanent housing needs and manage lease agreements.
    • Prepare subsidy applications, maintain client housing files, and ensure rent calculations are accurate.
    • Assist in monthly tenant rent collection, transport clients to housing locations and help them secure required documents for housing placement.
    • Conduct housing screenings and assessments utilizing the coordinated entry system required.
    • Shift coverage including part of the on-call team.
    • Provide field-based services as required by program and client need.
    • Maintain confidentiality of client’s care.
    • Participate in scheduled program meetings.
    • Other relevant duties as assigned.

QUALIFICATIONS

  • Bachelor’s degree and 1 year experience or associate degree and 3 years' experience or 5 years' relevant experience.
  • High School Diploma or GED required.
  • Valid CA driver’s license and current auto insurance per contract requirements required.
  • Knowledge of housing needs of individuals with behavioral health needs preferred.
  • Cultural competency understanding required.
  • Bilingual preferred.
  • Knowledge of Microsoft Office and EHR software preferred.
