Social Worker CSW / LSW (Nursing Home)

Hunterdon Healthcare and Rehabilitation is a sub-acute and long-term care facility located in Flemington, NJ is seeking a Full-Time CSW/LSW Social Worker to join our team of professionals.

Duties include, but are not limited to:

• Admission and Ongoing Assessments/Documentation

• Helping with Adjustment

• MDS 3.0

• Care Planning

• Assisting with Medicaid Applications and other Financial Matters

• Advocacy

• Crisis Intervention

• Discharge Planning

To qualify for this position, the ideal Candidate:

• Must have a NJ Social Work Certification CSW/LSW License in good standing

• Must be able to multi-task and organize

• Must have a passion for the senior population

• Must have strong writing skills

• Must be an effective communicator

• Must be a team player

Familiarity with Point Click Care (PCC) is a plus but not a requirement.
