Position Summary
We are seeking a highly organized and tech-savvy Care Manager / Inventory Specialist to join our growing team. This hybrid role combines the responsibilities of a traditional care manager with operational oversight of our medical device inventory systems. The ideal candidate is detail-oriented, reliable, and equally comfortable supporting patients and managing logistics.
Key ResponsibilitiesCare Management Duties
- Conduct regular patient outreach for remote patient monitoring (RPM) and chronic care management (CCM) programs.
- Document patient interactions, vitals, and updates in TOCA Health’s electronic system.
- Coordinate care with providers, nurses, and clinical leadership.
- Monitor patient compliance and escalate non-compliance issues as needed.
- Provide excellent patient service, empathy, and professionalism in all communications.
Inventory and Operations Duties
- Oversee all aspects of TOCA Health’s medical device inventory (blood pressure monitors, scales, pulse oximeters, etc.).
- Track, monitor, and reconcile inventory across patients and locations.
- Manage device delivery, return, cleaning, refurbishment, and redeployment.
- Serve as the primary contact for device setup, troubleshooting, and technical support.
- Maintain accurate logs of device assignments and replacements.
- Coordinate with vendors and internal departments to ensure timely replenishment and shipment.
Qualifications
- Minimum of 1–2 years in healthcare, care coordination, or medical office administration.
- Strong organizational and multitasking skills.
- Proficient in technology, including EMR/EHR systems, spreadsheets, and device connectivity.
- Excellent written and verbal communication skills.
- Experience with remote patient monitoring or medical inventory management is a plus.
- Compassionate, dependable, and able to work both independently and collaboratively.

Base Pay: From $22.00 per hour
