RN Care Coordinator

StartDate: 11/3/2025 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Care Coordinator

POSITION DUTIES – Care coordination / discharge planning / utilization review

Minimum Required Qualifications –

• CA RN license
• BLS
• Minimum of 3 years hospital experience and needs acute level care coordination experience
• Experience with discharge planning and utilization review
• Experience and proficiency with Midas, InterCall, Cerner, Curaspan, and Microsoft Office programs
• Prior CA Dignity/Commonspirit acute care experience
  
  

PREFERRED QUALIFICATIONS – Minimal Time Off Requests.

LENGTH OF ASSIGNMENT – 13 Weeks

SHIFT / HOURS PER WEEK – 8hr Days, EOW

SYSTEMS – Cerner

START DATE – ASAP
