Position: Registered Social Worker
Location: 1100 Las Tablas Rd, Templeton, CA
Shift: Days | Part-Time (24–32 hrs/week) Days (8:00 AM – 4:30 PM)

Job Description:

The Registered Social Worker provides psychosocial assessments, counseling, discharge planning, and support to patients and families coping with illness, hospitalization, and treatment. Works closely with the healthcare team to ensure coordinated care and may serve as a team leader within the department.

Key Responsibilities:

• Conduct assessments and develop care/discharge plans.
• Provide counseling and crisis intervention.
• Collaborate with medical teams for patient-centered care.
• Connect patients with community and social resources.
• Maintain accurate documentation per hospital standards.

Qualifications:

• Education: Master’s in Social Work (Required)
• Experience: 1 year in acute or long-term care (Preferred)
• License: Licensed Clinical Social Worker (LCSW) – Required in CA
