The Care Coordinator is responsible for delivering whole-person, client-centered services to individuals interested in participating in case management services. This includes conducting one-on-one meetings, providing case management support, and utilizing evidence-informed interventions that promote behavioral, emotional, and well-being. The Care Coordinator also assists clients in identifying and achieving their personal goals, while ensuring access to a full range of support services that may extend beyond mental health care. This position involves regular collaboration with internal and external partners and plays a key role in building and maintaining strong community partnerships to connect clients with essential resources and services.

Essential Functions/Responsibilities:
  • Provides and coordinates a continuum of treatment services and connection to community resources.
  • Receives internal and external referrals and carry a caseload of clients providing brief supportive counseling and concrete services.
  • Supports intake operations at Welcoming Center by conducting initial assessments, crisis interventions, treatment plan reviews, and other supportive services.
  • Operate within a multidisciplinary setting with effective communication between supervisory staff, collateral contacts and agencies, and other members of the team for the treatment and wellbeing of the clients.
  • Comply with productivity standards.
  • Carry a client caseload as developed.
  • Collaborate with clients and/or colleagues to complete IM+CANS, treatment plans, and other required documentation
  • Understand and follow all compliance rules regarding treatment of services and contracts.
  • Travel to at least two hospital sites to participate in multidisciplinary meetings, with the potential for expansion to additional locations as needed.
  • Attend community meetings with partner organizations to support collaboration, strengthen relationships, and ensure continuity of care across systems.
  • Adhere to all timelines regarding documentation completion.
  • Demonstrate sensitivity and responsiveness to cultural differences of clients, coworkers, and community.

  • Respond to all incoming correspondence and questions in a timely manner; including but not limited to inquiries received via email, telephone, text message, and fax.
  • Ensure reasonable measures are taken to protect confidential and/or sensitive information as well as ensuring the security of all LSSI systems is maintained.
  • Responsible for completing all required training assignments as well as complying with the related organizational, administrative, and programmatic policies.
  • Perform all other duties and tasks as assigned.
Position Qualifications:
  • Bachelor’s degree required.
  • Master's degree in a social service field preferred.
  • Background check clearance required.
  • LPC/LCPC or LSW/LCSW preferred.
  • Valid driver's license, in good standing for the state of residency required.
