We are seeking a compassionate and dedicated Outpatient Dialysis Social Worker to join our multidisciplinary team. In this vital role, you will provide psychosocial support including counseling and case management, education and advocacy for patients with chronic kidney disease receiving dialysis treatment on an outpatient basis or in the home setting. You will collaborate with medical professionals to ensure holistic, patient-centered care and help patients and their families navigate the complexities of chronic illness management. You will complete assessments and surveys per Mayo Clinic Policy/Procedure and CMS requirements and participate in dialysis clinic quality improvement processes.
To be considered for this position, you must have a Master of Social Work (MSW) or Master of Science in Social Administration (MSSA) degree, and have LCSW licensure to practice independently.

- This position is not eligible for visa sponsorship with the exception of the TN visa classification; Also, Mayo Clinic DOES NOT participate in the F-1 STEM OPT extension program.
To be considered for this position, you must have a Master of Social Work (MSW) or Master of Science in Social Administration (MSSA) degree, and have LCSW licensure to practice independently.
Additional Qualifications
- Advanced knowledge of mental illness, personality development, and the types of treatment, and skills in conducting individual, group, and family therapy.
- Knowledge of adjustment to medical diagnosis and chronic disease.
- Strong communication skills to interact with patients, patients' family members, consultants, residents, hospital personnel, community agencies, and other resources.
- Ability to communicate effectively in both verbal and written form.
- Psychosocial diagnostic skills.
- LCSW license to practice independently at the clinical level as required by applicable state requirements.
