Summary
The selected Personal Care Services (PCS) Social Worker will work within Social Work Service and provides services to Veterans receiving non-institutional care (NIC) through the Orlando VA Healthcare System. They will be responsible for the administrative an clinical reviews of Veterans receiving A authorized care in the community for one or more of the PCS programs, such as; Homemaker/Home Health Aide (H/HHA), Home Respite, Community Adult Day Health Care (CADHC), and/or Veteran Directed Care.
Duties
The selected Social Worker Personal Care Services (PCS) will work within Social Work Service and will provide services to Veterans receiving non-institutional care (NIC) through the Orlando VA Healthcare System. They will be responsible for the administrative and clinical reviews of Veterans receiving VA authorized care in the community for one or more of the PCS Programs, such as; Homemaker/Home Health Aide (H/HHA), Home Respite, Community Adult Day Health Care (CADHC), and/or Veteran Directed Care. Re-certification of program services may be completed via Veterans Video Connect (VVC), telephone, or other approved method. Services include provision of high-quality case management, clinical assessment, advocacy, and coordination of linkages to other appropriate VA or community service providers and/or agencies as needed by the Veteran. This is accomplished in collaboration with other members of the interdisciplinary treatment team. Some of the duties include, but are not limited to;
- Resolving requests to change assigned agencies and reviewing requests for additional services.
- Responsible for assuring that the documentation for all Veterans receiving VA-purchased home health care services have their need for service reviewed per-guidance outlined in the Standard Episode of Care (SEOC); e.g. every 180 days.
- Will act as liaison between VA and community home health agencies.
- Responsible for overseeing the recertification of services for Veterans admitted to the PCS Programs by the PCS Coordinator, ensuring Veterans re-authorized for services continue to meet program eligibility, e.g. H/HHA, CADHC, In-Home Respite, and VDC programs.
- Works independently in providing appropriate documentation, review, and follow-up of complaints within the PCS Programs.
- Responsible with the GEC Social Work Supervisor and PCS Coordinators for organizing and preparing reports concerning the PCS Programs for the Medical Center Director, VISN Liaison, and Central Office.
- Knowledgeable of Veterans' benefits and services relating to special programs, service-connected compensation, and nonservice-connected pension, including co-pay criteria for PCS Programs.
- Provides both informal and formal education to VA staff on any changes in VA policy concerning their programs, as well as community agency staff.
- Establishes rapports with agency/community providers, to monitor Veterans' care plans for re-authorization of services per SEOC.
- Enters all Veteran/family contacts in the electronic medical record using appropriate formats and templates.
- The participation of contributing to the successful completion of clinical reminders as appropriate for Social Work scope of practice and/or privileges and performance measures of the facility as assigned.

Basic Requirements:
- United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- Education: Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. - Licensure: Must be licensed or certified by a state to independently practice social work at the Master's Degree level. Current state requirements may be found by going to . Social Worker, GS-9
- Experience, Education, and Licensure: None beyond the basic requirements.
- Experience and Licensure: Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level. In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting.
- Experience with Geriatrics & Extended Care programs and services provided in the community to assist Veterans with aging in place, such as: Homemaker/Home Health Aide, Home Respite, Community Adult Day Health Care and Veteran Directed Care programs.
- Experience in performing assessments, treatment planning, care coordination, crisis intervention, and advocacy.
- Competency of care needs related to geriatric and Veteran populations.
- Experience in locating and obtaining VA and community resources specific to this group and assisting with linkage to appropriate referrals.
- Able to work independently providing psychosocial support to client populations with highly complex health and/or mental health problems.
The full performance level of this vacancy is GS-11. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-09 to GS-11.
Physical Requirements: The position requires performance of work which involves light physical effort, such as that involved in lifting light files, bending, standing, reaching, operating office equipment such as telephone, copier, fax machine, scanner, etc. The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty.
