Job Summary:

In this position, the Social Worker will provide comprehensive psycho-social assessment, management and support to at risk patients and families at Upstate Medical University Hospital. This position provides coverage to the entire downtown campus during evening hours with weekend rotation coverage to support throughput in the Emergency Department and inpatient units during high volume times. The Social Worker will actively manage patients through collaboration, coordination, monitoring, evaluating options and identifying required services to meet individual psychosocial needs. Candidate must possess strong written and verbal communication skills, have knowledge of available community resources, social determinants, population health and drivers of utilization. Candidate shall learn to initiate and implement advance directives. Duties will include consultation through assessment and documentation of brief counseling, interventions, discharge coordination and referrals to meet the psycho-social needs of patients and families. The Social Worker will participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and back-up responsibilities.

Minimum Qualifications:

M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. *New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Must obtain L.M.S.W. within 12 months of receipt of limited permit.

Preferred Qualifications:

Post MSW relevant experience.

Work Days:

Evenings with every other weekend rotation. Includes occasional on-call/holiday coverage on a rotation based on operational needs.
