As a member of the care team, the Transplant Intake Coordinator supports quality, patient, and family centered care. The Intake Coordinator is responsible for processing and documenting referrals, completing intake calls, scheduling of evaluations, and coordinating and participating in evaluations as needed. The Intake Coordinator will communicate closely with the RN Transplant Coordinator to provide a full continuum of care for the patient

Job Overview

The Intake Coordinator is responsible for processing and documenting referrals, completing intake calls, scheduling of evaluations, and coordinating and participating in evaluations as needed. The Intake Coordinator will communicate closely with the RN Transplant Coordinator to provide a full continuum of care for the patient
Responsibilities
Answers inquiry calls in a respectful manner and assesses special needs and/or accommodations.
- Coordinates and facilitates inquiry calls regarding the transplant program.
- Uses excellent telephone etiquette and active listening skills.
- Documents calls
- Assesses and prioritizes calls.
- Provides client or referral sources with information regarding the programs
- Educates patient on the referral and evaluation process
- Documents complete and accurate demographic and clinical information.
- Secures information pertinent to the patient visit.
- Works in conjunction with transplant schedulers, transplant administrative staff and coordinator.
- May be directed to schedule appointments for evaluation across multiple disciplines and service lines.
- Assists with preparation of necessary paperwork for transplant candidate and potential living donor patient
- Prepares records for insurance companies for financial clearance.
- Tailors evaluation to individual patient need and acuity
- Performs other duties as assigned in their scope of practice under the direction of the transplant coordinator
- Consults with appropriate resources regarding unique referrals or patient-specific needs
- Identifies issues that arise with patient access, throughput, quality, or efficiency and proactively notifies manager with possible solutions
- Communicates with patients when concerns arise regarding their experience in the system.
- Works in partnership with referring providers
- Acts as a liaison between patient/family, healthcare providers, referring physicians, and other hospital service lines.
- Partners with appropriate service line representatives to ensure proper work up and completion of evaluation.
- May accompany patient to various appointments, serves as liaison, ensures appropriate care at each phase of evaluation as directed by leader
Required Qualifications
- 2 years clinical experience in outpatient or inpatient environment.
- Computer proficiency
- Bachelor of Arts
- Graduate of an accredited post-secondary practical nursing program
- Experience with high acuity patient populations
