Part-time, 30 hours per week position.
Provide mental health screenings and referrals so that seniors suffering from anxiety, depression, and other mental health concerns get the help they need.
Resource for the other seniors in need of advocacy for social justice and elder abuse, to help them manage the emotional challenges associated with aging, and to answer questions about navigating care needs and community services.
Work alongside existing Municipal Agents in order to best meet the needs of our seniors and provide mental health services.
Fully licensed in the state of CT with specialized training and experience in treating older adults.
Complete any state mandated continuing education during their employment with the Estuary Council of Seniors.
Maintains electronic records utilizing appropriate software and maintains confidentiality of records and information in compliance with HIPPA.
Maintains and continuously seeks information on agencies and organizations that serve our nine-town residents. Participate in social service networking meetings and events.
Conducts outreach to connect vulnerable populations to activities, programs, resources, services and referrals to enhance overall quality of life. Makes presentations to at-risk residents and Town and Community service providers.
Provides center-based and community-based assessments to older adults to assess the level of functioning and needs in order to make referrals and coordinate services and resources.
Make reports and/or referrals to DSS of elder abuse when appropriate.
GENERAL DESCRIPTION:
The Human Services Social Worker works in the Community and Senior Services Department and provides social work services to a variety of age groups and populations (including frail older adults).
Works under the general supervision of the Executive Director.
Represents the Center in a professional and courteous manner.
Observes safe working practices.
Performs related work as required.
KNOWLEDGE, SKILLS AND ABILITIES:
Knowledge of presenting issues, stressors and developmental needs.
Skill and ability to perform general social work practice including engagement, assessment, intervention, monitoring and evaluation.
Knowledge of the issues and needs of the community for the senior population and the resources available to meet those needs.
Knowledge of available social service programs for referrals.
Experience with CharityTracker or similar HIPPA compliant tracking software.
Knowledge of Title IIIB grant process a plus.
Ability to demonstrate good judgment, empathy, sensitivity, flexibility and cultural responsiveness.
Ability to work independently within the community as well as cooperatively with department staff and other organizations and agencies.
Excellent written and communication skills needed.
Must possess excellent time management skills and ability to triage and prioritize cases.
Bi-lingual a plus.
QUALIFICATIONS:
Bachelor degree in Social Work or a related field, Masters Degree preferred. Minimum of 2 years work experience with populations including older adults, disabled adults, Veterans and/or financially at risk individuals and families.
Ability to travel locally required.
