Accountable and responsible for the following results:

  • Develop individualized transition plans in alignment with students’ IEP goals, strengths, and career interests.
  • Coordinate vocational assessments, job readiness training, and work-based learning opportunities.
  • Establish and maintain partnerships with local businesses, agencies, and community organizations to create employment and training opportunities for students.
  • Provide direct instruction in career exploration, workplace readiness, and independent living skills.
  • Oversees and updates student academic files to ensure accurate credit tracking, grade reporting, and transcript evaluations.
  • Monitor student progress and adjust transition services as needed.
  • Collaborate with teachers, related service providers, and families to ensure alignment of transition goals and services.
  • Support students in accessing community resources, vocational rehabilitation, and post-secondary education/training programs.
  • Maintain accurate records and complete required documentation in compliance with federal, state, and district regulations.
  • Facilitate student participation in Extended School Year (ESY) programming when appropriate.
  • Provide direct instruction in vocational skills aligned with student transition goals.
  • Coordinate and oversee student participation in on-campus and community-based work experiences.
  • Supervise and evaluate daily work crews as part of the TSW/DTP program.
  • Monitor student performance at work sites and maintain quality standards for all vocational tasks.
  • Serve as a case manager for assigned students, developing and implementing Individualized Education Plans (IEPs) related to vocational goals.
  • Maintain accurate student records, including attendance, progress reports, and job evaluations.
  • Communicate regularly with families to support transition planning and post-secondary readiness.
  • Collaborate with job coaches and paraprofessionals, providing training and instructional support.
  • Ensure all necessary equipment, tools, and supplies are available and maintained for the vocational program.
  • Use effective classroom and behavior management techniques to maintain a positive learning environment.
  • Manage payroll documentation for student workers and track income/expenses related to student-run work crews.
  • Attend and support vocational training outreach at community festivals and events.
  • Participate in team meetings, IEP meetings, professional development, and transition planning sessions.
  • Completes and maintains all agency-mandated trainings within required timeframes to ensure compliance with organizational, state, and federal standards.
  • Maintains appropriate personal boundaries with young people and families served at The Children’s Center of Hamden, in accordance with the agency’s Boundary Code of Ethics.
  • Maintains awareness of The Children’s Center of Hamden’s Policy and Procedure Manuals and accesses relevant information as needed to guide decision-making and ensure compliance.
  • Contributes to a safe, welcoming, and equitable environment that reflects the agency’s commitment to diversity and inclusion.
  • Ensures the physical and emotional well-being of students by maintaining a safe, secure, and nurturing environment.

Job Type: Full-time

Pay: $50,000.00 - $60,000.00 per year
