Key Responsibilities:

• Assess patients' mental health needs.
• Provide support services to patients and their families.
• Maintain accurate and timely clinical documentation.

Position Qualifications:

• Social Work license.
• Reliable transportation and valid auto insurance.
• Strong clinical abilities, effective communication, dedication to quality patient care.
• Prior home health experience preferred; training available for suitable candidates.

Hours: Monday – Friday, flexible hours on a PRN basis.

Work Type: Hybrid / traveling within a reasonable distance to patients' homes.

Pay: $40 - $45 per hour.
