As Central Intake Coordinator you must meet the following qualifications:

• Previous experience as a Central Intake Coordinator or similar role
• Prior knowledge of skilled nursing industry preferred
• Superior customer service, communication and interpersonal skills
• Experience in data entry and record keeping
• Proficient in referral portal systems such as Watershed, Epic, NaviHealth, etc.
• Proficiency in MS Office and database software
• Ability to handle HIPPA and other protected patient information with discretion
• Minimum associate’s degree or equivalent. Higher degree in social work or a related field is a plus
