What you’ll be doing:
Care Coordinator will help with weekly and daily scheduling, client intakes, develop individual service plans, document all activities in WellSky, new caregiver meet and greets, caregiver on site training, caregiver evaluations, client quarerly evaluations, rotating on call weekends/after hours, and any activities that involve the day to day care of home care clients.
Must be able to commuctate on a high level to ensure 100% client satifaction ratings and best in class customer services. Must have a possitive attitude and work well as a team with other care coordinators to ensure best in class home care service.
Must have previous care coordinating experience.
- Assist with daily activities to help clients stay independent and in their own homes
- Provide assistance with personal care
- Assist with mobility, transfers and range of motion exercises
- Handle meal planning, preparation and feeding
- Properly manage household needs
- Provide companionship and respite services for the family.
- Must have reliable transportaion as you will be working with clients in Hammond, Covington, Mandeville, Slidell, the entire North Shore and Surrounding areas.
- This is a full-time position.
What we’re looking for:

- 18 years or older
- High school diploma or G.E.D. certificate
- Access to reliable transportation and valid driver’s license
- Must be able to pass a background check and pre employment screening
- Previous experience as an at home caregiver or in senior communities
- Must have a possitive attitue and a sencere desire to help care for others
