Med Staff Home Health is Hiring a Community Liaison!

Position Summary

As a Community Liaison, you will build strong relationships with referral sources and community partners to increase awareness and utilization of our home health services. You’ll serve as the face of our practice—educating providers, facilities, and families about the compassionate care we provide.

Schedule

● Full-Time

Key Responsibilities

● Develop and maintain relationships with referral sources (e.g., physicians, hospitals, skilled nursing facilities, assisted living, case managers, etc.)

● Promote awareness of home health services through in-person visits, presentations, and community events

● Identify new referral opportunities and help guide families through the referral/admission process

● Collaborate closely with clinical and leadership teams to ensure a seamless patient experience

● Track marketing efforts, analyze trends, and adjust outreach strategies accordingly
