WHO WE ARE: Magnificat Houses, Inc. Since 1968, Magnificat Houses has provided an alternative to the streets by sheltering, clothing, feeding, and providing mental health support and spiritual guidance to men and women as they rebuild their lives in community.
Role and Responsibilities
The clinical social worker is responsible for providing various supportive services to chronically homeless and disabled adults, matched to the project to promote long-term stability, self-sufficiency, and eventual movement into mainstream housing.
- Master’s degree in social work
- 5+ years of clinical social work experience
- Valid and current LCSW or LMSW license
- Excellent listening and communication skills
- Ability to make psychosocial assessments and develop and implement care plans
- Evaluate participants based on their needs, limitations, and desires
- Assume responsibility for data management and reporting; enter complete and accurate data into HMIS
- Maintain complete and accurate participant files with current case notes and supporting documentation
- Exhibit and maintain current knowledge of community resources, make referrals to the appropriate community partners
- Assist participants with developing a movement towards mainstream housing by increasing income and housing stability
- Address participant concerns and goals while maintaining constant communication with the participant and the case manager/services team
- Develop care plans for participants with intense needs
- Facilitate community partner referrals and conduct follow-up services for referral outcomes
- Maintain accurate and timely client hard copy and digital file information
- Perform other duties as assigned
Qualifications and Education Requirements
- Master’s degree in social work
- 5 or more years of relevant experience

- Supportive housing
- Experience with HMIS
- Knowledge of the chronically homeless population and best practices
Additional Notes
This position reports to the Social Work Sr. Manager and is a key contributor to the overall success of the housing team.
