Dignity Health Connected Living has been serving seniors in Shasta County since 1968. Our programs have been designed for seniors and people with disabilities. We are dedicated to meeting each participant's physical, mental, and spiritual needs and we offer a variety of high-quality, affordable health care services in a compassionate environment.

Responsibilities:

Under direct supervision the incumbent identifies and evaluates potential clients as total person with focus on health limitations which impede independent living. The incumbent will certify level of care for the clients, provide medical information and training to care management staff, participate in California Community Transitions program and assist the development of organizational programs in response to changing health care services

Qualifications:

Minimum qualifications

• Three (3) years experience two (2) of which are in public health
• Associate of Science degree in nursing
• Valid current California license to practice nursing

• Geriatrics experience
• Bachelors degree in nursing
