Qualifications:

• MSW required (licensure preferred).
• 2+ years of social work leadership/supervision (healthcare/elderly population experience strongly preferred).
• Minimum 1 year of experience with frail/elderly populations.
• Strong clinical, organizational, and communication skills.
• Valid driver’s license, clean DMV record, and current auto insurance.\

Key Responsibilities:

• Lead and supervise Social Work Supervisors, Social Workers, Assistants, and Behavioral Health Specialists (hiring, training, coaching).
• Oversee social work assessments, care plan reviews, documentation, and compliance.
• Ensure delivery of social work interventions, urgent care needs, and participant rights processes.
• Collaborate on behavioral health programming, including support groups, counseling, and family education.
• Participate in end-of-life care planning and occasional home visits.

Job Type: Full-time

Pay: $107,000.00 - $141,000.00 per year
