Overview:

The need for additional Clinical Care Coordinators arises due to the growing demand and complexity of managing patients within Remote Patient Monitoring. These positions are crucial to ensure the effective coordination and delivery of care to our patients, improve patient outcomes, and enhance the overall efficiency of our healthcare services. The addition of skilled coordinators will provide the necessary support to manage patient cases, facilitate communication across care teams, and ensure the smooth execution of the programs.

Duties:

Clinical Care Coordinator will be working in Remote Patient Monitoring Program and the duties will be following

Monitoring and Data Review:

· Regularly review patient data collected from remote monitoring devices.

· Monitor and identify any trends, abnormalities, or issues in patient data.

· Flag critical data points for follow-up with healthcare providers.

Patient Communication:

· Contact patients regularly to discuss their health status and answer any questions regarding

the RPM system.

· Provide reminders to patients about regular usage of the monitoring devices and encourage

consistency in reporting.

· Address any concerns or issues patients may have regarding the devices or their health.

Troubleshooting and Technical Support:

· Assist patients with troubleshooting technical issues related to the RPM devices or the

mobile app.

· Ensure patients understand how to properly use the devices, including cleaning, storage,

and recharging (if applicable).

· Coordinate with Care Manger if problems are beyond the Care Coordinator’s scope of

resolution.

Coordination with Healthcare Providers:

· Share relevant patient data with physicians, nurses, and other healthcare providers to inform

their treatment decisions.

· Ensure healthcare providers are notified about critical changes in the patient’s health data

that require intervention.

Experience:

1. 1-2 years of direct patient care experience

2. Experience in Care Coordination or Case Management

3. Experience with Remote Patient Monitoring

4. Familiarity with EHR
