We are seeking a Case Manager to monitor in-home services provided to consumers receiving Consumer-Directed Waiver Services to ensure the health, safety, and well-being of the consumer while the consumer remains independent in the community. Open positions in Lynchburg, Forest, and Campbell County. Position is part-time with a flexible schedule. The rate of pay is per completed visit.
Responsibilities and Duties:
· Complete monthly in-home visits
· Document visits using designated DMAS documents and the company’s CSM database
· Identify needs and make necessary referrals
· Support the consumer in developing and monitoring a Plan of Care for personal and respite care
Qualifications:
· Minimum of 2 years of verifiable experience working with the elderly and/or disabled
· Ability to communicate with consumers, providers, and agencies
· Good organization and time management skills.
· Experience with Microsoft products
Requirements:
· Reliable transportation and a valid Driver’s License (must conduct face-to-face visits)
· Successfully pass 2 background checks with no barrier crimes
· Access to internet in home as work from home is required
Job Type: Part-time
Pay: $20.00 - $25.00 per hour
