Job Summary
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will provide support and advocacy for individuals and families facing various challenges, including mental health issues, addiction, and crisis situations. This role involves working closely with clients to assess their needs, develop treatment plans, and facilitate access to resources. The Social Worker will also collaborate with healthcare professionals to ensure comprehensive patient care.

Duties

  • Conduct thorough patient assessments to identify needs and develop individualized care plans.
  • Provide clinical counseling and therapy services, including individual and group counseling sessions.
  • Implement evidence-based practices such as cognitive behavioral therapy and motivational interviewing.
  • Facilitate crisis intervention and management for clients in distress.
  • Maintain accurate medical documentation in compliance with HIPAA regulations.
  • Collaborate with multidisciplinary teams in hospitals, outpatient clinics, or assisted living facilities.
  • Engage in case management activities to coordinate services for clients.
  • Conduct intake assessments and diagnostic evaluations for new clients.
  • Participate in clinical research initiatives to enhance service delivery and patient outcomes.
  • Support patients with addiction counseling, hospice care, or behavioral health challenges.
  • Provide education and resources to families regarding mental health issues, dementia care, PTSD care, and developmental disabilities.

Qualifications

  • Master’s degree in Social Work (MSW) from an accredited institution is required.
  • Valid state licensure as a Clinical Social Worker (LCSW) or equivalent is preferred.
  • Experience in child & family counseling, geriatrics, pediatrics, or working with individuals with disabilities is highly desirable.
  • Familiarity with EMR systems and medical terminology is essential for effective documentation.
  • Strong understanding of clinical practices including crisis management, behavioral therapy, and group therapy techniques.
  • Knowledge of ICD coding (ICD-9 & ICD-10) for accurate medical record keeping is beneficial.
  • Excellent communication skills with the ability to build rapport with diverse populations.
  • Ability to work collaboratively within a team-oriented environment while maintaining client confidentiality.
