The Family Counseling Center of Fulton County, Inc. is dedicated to delivering high-quality behavioral health services. We are currently seeking a Care Coordinator to join our team and help ensure clients receive the comprehensive care and support they need to achieve their treatment goals.
Job Summary:
The Care Coordinator will work closely with clients, families, healthcare providers, and community resources to develop and monitor personalized care plans. This role ensures clients access the services they need, such as mental health treatment, substance use support, and other health-related services, while advocating for their needs and navigating the healthcare system.
Key Responsibilities:
- Conduct comprehensive assessments of clients' health and social needs.
- Develop and update care plans in collaboration with clients, their families, and a multidisciplinary team.
- Coordinate access to mental health, substance use treatment, social services, and other resources.
- Advocate for clients’ needs within the healthcare system.
- Provide education and support to clients and families about services and treatment options.
- Respond to crises and provide follow-up support.
- Maintain accurate documentation in the electronic health record (EHR) system.
- Ensure compliance with CCBHC standards, state and federal regulations.
Qualifications:
- Bachelor's degree in Social Work, Psychology, Nursing, Public Health, or a related field (Master’s degree preferred).
- MSW, MHC, or RN preferred
- Minimum of 2 years of experience in care coordination or a related field.
- Experience working with individuals with serious mental illness and substance use disorders.
- Strong understanding of behavioral health, substance use, and social determinants of health.
- Excellent organizational, communication, and interpersonal skills.
- Proficient in EHR systems and care management software.
- Ability to work independently and collaboratively in a team.
- Commitment to cultural competence, equity, and professional growth.
