Southern Tier AIDS Program, Inc. is looking for a Health Homes Plus Care Manager to join our Ithaca team! This position provides confidential outreach, intake, assessment and highly intensive care management services for designated individuals as part of the Medicaid Health Homes program. Individuals enrolled in Health Homes Plus are eligible for a higher level of support for up to 12 consecutive months. The Health Homes Plus Care Manager reports directly to the Health Homes Program Coordinator.
Responsibilities
The primary responsibilities and duties of a Health Homes Plus Care Manager include:
- Maintains caseload ratio for HH + enrollees as indicated by DOH policy. The HH + care manager must provide a minimum of four (4) Health Home core services per month, two (2) of which must be face-to-face contacts, or more when the individual’s immediate needs require additional contacts.
- Provision of outreach, intake, and intensive care management services for defined populations diagnosed with HIV/AIDS or a Serious Mental Illness, their families, and collateral contacts receiving services.
- Serves as an agency and/or client liaison/advocate with health and human service providers throughout the continuum of care and community.
- Provides linkages, referral, advocacy, and assistance with information including but not limited to: chronic disease management, benefits related to treatment adherence, benefits/entitlements, HIV-disease, harm reduction, etc. As such care managers are responsible for establishing contacts and relationships with individuals and providers in their designated area of responsibility.

$20.61/hour working 35-hours per week, may vary depending upon experience
Education:
- Bachelor’s degree in Community Mental Health, Social Work, Nursing, Psychology, or Sociology and two years of qualifying experience.
Experience:
- Must have a minimum of two years’ experience providing direct services to persons with Serious Mental Illness or chemical dependency.
- Qualifying experience means: verifiable work with the target populations: individuals with HIV, history of mental illness, homelessness, or substance use.
- Organizational skills, record-keeping, time management and communication skills are essential.
- Computer literacy required.
- Physical demands are those associated with general office duties including: sitting, standing and light lifting. This individual will travel in the community (i.e. visiting with service providers, client home visits, etc.).
- This individual is responsible for meeting the transportation and insurability requirements of this job. A valid driver’s license, clean driving record and reliable transportation is a requirement of this position.
