Position: Supervising Care Manager (SCM)
Job Type: Full-Time 40 Hours per week
Salary: $32.00 -$34.25 /hr.
General Description:
Under the direction of the Site Director, the SCM is responsible for planning, organizing, developing and implementing the principles and techniques employed in a comprehensive care management model for MSSP older adults. The SCM position combines both administrative duties and clinical supervision responsibilities. The SCM is responsible for providing clinical oversight to ensure all care management activities and ensures documentation meets CDA Standard Agreement Requiremenst and Waiver requirements. While both areas have to be balanced, it is important that the time required for direct clinical supervision of the care managers not be eroded by administrative and managerial functions.
Major Responsibilities/Duties:
Ensures that client waiver costs are cost effective and are within the benchmark set by the California Department of Aging (CDA). Reviews all SWCM's purchase orders against MSSP Care progress notes prior to submission to finance.
1. Ensures that waiver funds remain within budget as set by the Catholic Charities finance department.
2. Provides ongoing supervision of the care management staff; assigns clients; reviews and approves all client care plans, tracks monthly client count, prepares CIR and Quarterly reports for Site-Directors review and submission.
3. Handles client complaints and when appropriate refers the matter to the Site-Director.
4. Conducts weekly care plan meetings with staff. Arranges and provides monthly staff meetings and trainings.
5. Supervises the development and implementation of care management (procedures and techniques of interviewing MSSP clients; case recordings; conducting assessments/evaluations, developing service plans, developing and implementing program policies and procedures, and ensures compliance with HIPAA regulations.
6. Analyzes, interprets, and evaluates effectiveness of the California Department of Aging regulations, polices, procedures, and contracts; reviews cases to ascertain conformance with legal and procedural standards; serves as the information resource person for other staff members regarding specific services.
7. Reviews and develops interagency agreements/contracts relating to the provision of services for the approval of the Site Director and Finance Department.
8. Coordinates with other agencies and individuals to ensure effective delivery of services; participates in developing uniform guidelines and procedures.
9. Develops and participates in outreach, trainings, and public information on MSSP.
10. Ensures that individuals entitled to services are made aware of service programs; interprets the scope and limitations of departmental programs to recipients and others interested; researches and answers public inquiries and complaints.
11. Participates in statewide MSSP Supervising Care Managers meetings and MSA meetings.
12. Responsible for the overall Utilization Review (UR) process and UR Response to CDA staff.
13. Utilizes the program software "MSSP Care" effectively and trains the SWCM's on the use of MSSP Care. The MSSP SCM duties described above are supported by, but not restricted to other related professional duties.
Desired Qualifications:
Master’s degree in psychology, counseling, rehabilitation, gerontology or sociology and two years
experience working with older adults.
1. Master's degree in social work, nursing, psychology, counseling, rehabilitation, gerontology, or sociology is required.
2. Minimum of one-year supervisory experience required.
3. Minimum of two years experience working with the aging population (60 years old and above) required.
4. Minimum of one-year experience in care management in required.
5. Is knowledgeable of the services available to older adults in the San Joaquin County.
6. Strong interpersonal, organizational and communication skills.
7. Must be able to occasionally work after hours, overtime, and travel as needed to attend onferences, trainings, meetings (includes out-of-state travel).
8. Must have a current CDL, reliable, insured vehicle.
9. Fluency in both English and Spanish is highly desirable.
