We are seeking Qualified Mental Health Professionals (QMHPs) to join our team as Care Coordinators and Parent Educators. In this role, you will provide wraparound support to pregnant, postpartum families and their babies, combining mental health expertise with parent education and care navigation. You will play a key role in helping families access resources, develop parenting skills, and achieve greater stability and wellness.

Key Responsibilities

  • Caseload Management- Maintain an active caseload of expectant and postpartum mothers and their babies.
  • Early Engagement- Reach out to families as early in pregnancy as possible to begin building rapport.
  • Relationship Building- Establish a trusting, culturally sensitive relationship with each parent.
  • Home & Site Visits- Conduct individual visits in clients’ homes or at locations such as doctors’ offices and prenatal appointments.
  • Education & Support- Deliver one-on-one and group education on prenatal, postpartum, and newborn care, Provide lactation support as needed
  • Documentation- Complete and chart all required visit documentation
  • Continued Follow-Up- Maintain regular postpartum contact
  • Team Collaboration-Serve as a supportive member of the team under supervision. Act as a resource for other staff
  • Meetings & Training- Attend bi-weekly or monthly conference calls, office meetings, and all required trainings.
  • Community Outreach- Participate in community-based recruiting sessions and events to raise program awareness.
  • Confidentiality- Uphold strict confidentiality of all client information.

Eligibility:

  • Education & Experience- Bachelor’s degree (health or human services preferred) and 2 years of direct patient care. Maternal health background is a huge plus!
  • Schedule Flexibility- Comfortable working days, evenings, weekends, and on-call shifts
  • Relationship-Builder-Proven ability to earn trust with high-risk prenatal clients and team members
  • Home-Visiting Skills-Experience in home visiting or early childhood/Head Start programs (prenatal through postpartum) preferred
  • Women’s Health- Solid grasp of women’s health needs during childbearing years
  • On the Road- Reliable transportation and willingness to travel up to 45 miles across Norfolk, Newport News, Hampton, VA Beach, Chesapeake, and nearby communities
  • People-Person- Natural at forging respectful, supportive relationships with staff, families, and children
  • Communication- Strong facilitation, presentation, and writing skills
  • Passion- Experience working with communities of color and a commitment to racial, disability, gender, and economic equity
  • Creative Problem-Solver- Resourceful and imaginative when tackling challenges
  • Tech Fluent- Comfortable using computers and digital tools every day

Qualifications

  • Must be registered with the State of Virginia as a QMHP.
  • Bachelor’s or Master’s degree in social work, psychology, counseling, or related human services field.
  • Experience in maternal, child, or family health preferred.
  • Two years of solid work history in public health.
  • Strong knowledge of trauma-informed and family-centered practices.
  • Excellent communication, organization, and documentation skills.
  • Ability to work in diverse community, home, hospital, and telehealth settings.
  • Reliable transportation required.

This role is centered on home visits and community engagement rather than remote work.
