Summary:
Join our dedicated team of Care Managers that specialize in providing comprehensive care for aging adults. Our team works from home and provides chronic care management services for patients who are living in skilled nursing, assisted living, and other long-term care facilities.
Essential Duties & Responsibilities:
Work from home
Day shift hours, Monday-Friday
No more heavy lifting or responding to call lights.
Preferred Qualifications:
Familiarity with chronic care management and geriatric care.
Experience With Electronic Health Records And Other Healthcare Software.
Previous experience working in skilled nursing, assisted living, or long-term care settings.

Pay Range: USD $28.00/Yr. - USD $32.00/Yr. Pay is based on several factors including but not limited to education, work experience, certifications, etc. In addition to your salary, Curana Health offers benefits such as, a comprehensive benefits package, 401K, PTO, paid holidays (all benefits are subject to eligibility requirements).

Summary:

- Perform comprehensive chart reviews, reconcile medications, and identify potential issues that need to be addressed.
- Develop and implement personalized care plans tailored to each patient's needs.
- Monitor and assess patient progress, adjusting care plans as necessary to ensure optimal outcomes.
- Collaborate with other healthcare providers and facilities to coordinate patient care and ensure a cohesive approach.
- Document all patient interactions, assessments, and care plans accurately in the electronic health record system.
- Ensure compliance with all healthcare regulations and standards of practice.
- Must be a Licensed Practical Nurse (LPN) with a compact license in good standing.
- Strong communication and interpersonal skills.
- Proficient in using computers and various software applications.
- Self-motivated and able to work independently in a remote setting.
