The Community Liaison will require interactions with physicians, hospitals, and community partners as we seek to provide continued delivery of care throughout the service area.

Essential job functions & responsibilities
Your responsibilities would include communicating the benefits of hospice care to these professionals and families, educating the community about hospice services, and conducting sales calls to existing and potential referral sources.
Establish and maintain positive working relationships with current and potential referral and payer sources

  • Build and monitor community, customer, payer, and patient perceptions of the organization
  • Assist in strategic planning to identify opportunities for additional or improved services to address customer needs
  • Maintain comprehensive working knowledge in the field of marketing
  • Maintain comprehensive working knowledge of Care Plus Hospice markets
  • Extensive experience in healthcare sales, especially home care and/or hospice
  • Ability to market aggressively while simultaneously maintaining positive industry relationships
  • Demonstration of good communication, negotiation, and public relations skills
  • Ability to work independently
  • Ability to build and maintain relationships with referral sources
  • Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance

Qualifications:

Works well within a team environment to collaborate daily with interdisciplinary team.

Can work independently and on your own on a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.

Time management skills to manage visits, documentation, and meetings.

Ability to learn electronic medical records system.

Must have reliable transportation.

Continuously evaluates personal and professional performance and makes necessary changes to increase productivity and quality of care delivered.

Adheres to and supports the policies/procedures/goals/objectives of hospice to assure quality patient care.

Strives for independence and sound critical decision making

Job Types: Full-time, Permanent

Pay: $50,000.00 - $100,000.00 per year
