Role: Clinical Social Worker
Work Schedule: Full time - Monday through FridayKey Responsibilities:
- Conduct initial psychosocial assessments for new participants, including cognitive status, social support systems, mental health, and substance use issues.
- Regularly assess and reassess enrolled participants’ social, emotional, and psychological needs, and help develop comprehensive care plans in collaboration with the Interdisciplinary Team.
- Provide ongoing education, counseling, and support to participants and their families on topics such as aging, dementia, grief, end-of-life issues, and the PACE model.
- Act as a liaison between the team, caregivers, and community agencies to ensure participants' needs are met, and advocate for necessary resources and services.
- Facilitate meetings with families, caregivers, and other providers to solve care plan challenges and promote participant well-being.
- Coordinate hospice care services when necessary, providing emotional support and assisting with funeral and financial planning.
- Ensure smooth transitions for participants requiring temporary respites or permanent placements in contracted facilities.
- Master’s Degree in social work from an accredited school.
- Active and valid Maryland State licensure as Licensed Clinical Social Worker.
- At least 1 year of experience working with the frail or elderly population.
- Experience in a multi-disciplinary healthcare setting (hospital, nursing home, or community-based) is preferred.
- Must be able pass a comprehensive Background Investigation that may include Criminal, Employment, Education, / License and Drug Screen
- Ability to conduct thorough psychosocial assessments and develop comprehensive care plans.
- Strong communication skills for interaction with participants, families, team members, and external agencies.
- Experience in working with senior populations, particularly those with complex medical and social needs.
- Compassionate and empathetic approach to counseling and support.
- Ability to work collaboratively as part of a team.
