Social Workers will perform beside initial and ongoing case management assessments, counseling, referral and discharge planning services to all age groups of patients treated at Holy Cross; provides supportive services to patients and their families.

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

Position Purpose
Social Workers will perform beside initial and ongoing case management assessments, counseling, referral and discharge planning services to all age groups of patients treated at Holy Cross; provides supportive services to patients and their families.
What You Will Do
- Social Workers are expected daily to support all patient discharge transitional needs from the inpatient setting back to the community.
- Review clinical supporting documentation to support the throughput of all patients who need transitional care from the inpatient setting to the appropriate next level of care.
- Collaboratively support discharge planning by attending the daily IDRs on the units to coordinate the patients' discharge needs.
- Attend all IDRs, and other department meetings that support throughput.
- State of Maryland LSWA, LGSW, LCSW or LCSW-C licensure (or license eligible).
- Masters Degree in Social Work.
- Previous social work experience in an inpatient setting.
- Demonstrated ability to work with all age groups of patients, with an emphasis on adult and geriatric patients. Able to communicate effectively.
