Pennsylvania Hospital is looking for a PRN Social Worker for OB and the Intensive Care Nursery. Must have weekday availability and a minimum of 2 weekend days per month.
Summary:
Identifies and attends to the psychological needs of patients and families.
Provides counseling to patients, families and groups.
Responsible for documentation in the medical record, statistics and reports as required.
Facilitates legal matters, financial entitlements and complies with grants and contracts when applicable.
Maintains relationships with community agencies and keeps up-to-date on resources.
Uses effective interpersonal skills in all interactions.
Enhances own professional development and maintains formal and informal continuing education for self improvement.

Summary:
- Responsible for discharge planning and contributing to the patient's treatment plan through and understanding of social, psychological and/or environmental needs related to the impact of illness and need for hospitalization. Professional activities will be carried out in accordance with the code of ethics established by the National Association of Social Workers.
- Coordinates the discharge planning process to ensure a safe and timely transition from hospital to home, community, or institutional setting.

Credentials:
- Licensed Clinical SW (Preferred)
- Master of Social Work (Required)
- And 0-1 years Health Care experience in field instruction and/or employment in a health care setting.
- Prior OB experience (Preferred)
