JOB SUMMARY
The Housing Case Manager is responsible for the delivery of direct services to adult survivors of domestic violence participating in the YWCA Greater Cincinnati Housing and Supportive Services Program. Using a trauma-informed service model, the Housing Case Manager will assess client needs, provide support, and facilitate connections to needed resources to help survivors access and maintain housing in Hamilton, Clermont, Brown, and Adams counties.
PRINCIPAL DUTIES & RESPONSIBILITIES
  • Facilitates intake into the appropriate housing project, completes required paperwork, creates client files and enters data into the required database.
  • Develops and implements an individual case plan with each participant assigned by the Housing and Supportive Services Program Manager.
  • Works alongside participants to identify and access housing in the client’s desired geographic area within the counties of Hamilton, Clermont, Brown, or Adams County.
  • Maintains familiarity with community resources in each county and ensures the provision of up-to-date and viable information to assist participants with their needs. Develops and maintains strong relationships with housing providers and employment programs in each county.
  • Ensures provision of information related to domestic violence and safety planning for each resident assigned by the Housing and Supportive Services Program Manager.
  • Stays up to date on community and/or regulatory policies and ensures required policies/procedures are followed.
  • Works closely with Landlords to promote supportive housing and mediate problems when necessary.
  • Maintains appropriate client data and documentation in accordance with YWCA Greater Cincinnati policies and procedures.
  • In consultation with Housing and Supportive Services Program Manager, handles medical, psychiatric, and other emergencies.
  • Participates and presents updates relative to client progress/status at scheduled case review meetings.
  • May periodically provide transportation to participants to outside appointments relative to housing, legal or medical needs.
  • In consultation with the Housing and Supportive Services Program Manager, intervenes when conflict occurs between program participants and/or mediate conflict between program participants and landlords.
  • Coordinates exit planning and provide aftercare services to former program participants up to 6 mos. post-exit.
  • Maintains confidentiality of all participants, past and present, in accordance with program and agency policies.
SECONDARY DUTIES & RESPONSIBILITIES
  • Completes HQS and Habitability Standard inspections.
  • Assist Housing Specialist with monthly inspection of site-based transitional units.
  • Answers the business line.
  • Dispenses bus tickets.
  • Works with and provides on-the-job training to volunteers and student placements as assigned.

ADDITIONAL DUTIES & RESPONSIBILITIES
  • Communicates emergencies and other urgent matters to the Housing and Supportive Services Program Manager promptly.
  • Assists Housing and Supportive Services Program Manager with collection of statistical/programmatic information.
  • Meets regularly with Housing and Supportive Services Program Manager for supervision.
  • Attends mandatory staff meetings, in-services and training courses.
  • Networking in the communities we serve for collaborations, partnerships and resources for survivors fleeing domestic violence.
  • Other duties as assigned
CLERICAL/GENERAL ADMINISTRATIVE
  • Responds to routine emails, phone calls, etc.
  • Makes copies, prints documents, etc.
  • Electronically file paperwork.
  • Timely data entry to Clarity/Empower DB per the agency’s and funders’ policies.

Key Relationships
  • Interacts with Housing and Supportive Services Program Manager, YWCA staff, student placements, clients, landlords, and representatives from outside agencies.

QUALIFICATIONS:
Education/Experience
  • Bachelor’s degree in a related field preferred; equivalent combination of education and experience will be considered.
  • Experience in human services, women’s advocacy, trauma informed care and crisis intervention preferred.
Licenses/Credentials/Certifications
  • LSW preferred.
Skills/Specialized Knowledge/Abilities
  • Cultural competence and the ability to work effectively with people from diverse backgrounds.
  • Competence in case management planning and assessment and systems advocacy.
  • Ability to multi-task and work independently with little oversight. High level of integrity, professionalism, and self-motivation. Must be able to manage time effectively to ensure high quality service provision and recordkeeping.
  • Excellent written and verbal communication skills
  • Understanding of the dynamics of domestic violence and the need for flexibility, mature judgement and confidence.
  • Ability to work with individuals across the spectrum of gender, race, religion, ethnicity, national origin, sexual orientation and socioeconomic background.
  • Ability to maintain confidentiality in all settings.
  • Ability to reflect the YWCA mission to empower women and eliminate racism in work performance.
  • Knowledge of community resources in rural Ohio counties a plus.
  • Reliable transportation and the ability to travel within the service area.
  • Ability to work on-site at downtown YWCA 3 days per week.
WORKING CONDITIONS:
Working Hours/Environment
  • 37.5 hours per week. This position follow a hybrid model, requiring at least 3 days per week on-site at a designated YWCA office. Primary reporting office may not be the downtown location due to rural service needs.

  • Personal computer, copier, fax/scanner, phone, and other typical office equipment.
Travel
  • As required with respect to job responsibilities and business needs.
  • Frequent travel within Hamilton, Clermont, Brown, and Adams counties is required.
