Job Purpose:
To deliver compassionate, holistic social work services to hospice patients and their families—providing emotional guidance, spiritual support, crisis counseling, and advocacy, while connecting them to essential community resources.
Key Responsibilities:
- Perform initial and ongoing psychosocial, spiritual, and emotional assessments.
- Collaborate in developing patient-centered care plans.
- Provide individual and family counseling, including crisis intervention.
- Serve as a bereavement team member, offering grief support as needed.
- Act as an advocate for patients and families in IDG meetings.
- Maintain accurate and timely documentation of all services and contacts.
- Develop community relationships to enhance referrals and outreach.
- Participate in staff meetings and continued professional development.
Qualifications:
You may qualify for this role with one of the following:
- MSW from a CSWE-accredited school and one year of health care experience.
- BSW from a CSWE-accredited school + one year of experience in a health care setting (supervised by an MSW unless exempt).
- Bachelor’s in psychology, sociology, or related field + one year of health care social work experience (with MSW supervision).

Physical Requirements:
