We are looking for a compassionate and dedicated Care Manager to join our Developmental Disabilities Division in Buxton, ND!

As a Care Manager at All Embracing Home Care, you will have the opportunity to make a positive impact in the lives of individuals with developmental disabilities. You will be responsible for providing personalized care plans, coordinating services, and advocating for the needs of our clients. Your role will be crucial in ensuring that our clients receive the highest quality of care and support to help them live fulfilling and independent lives.

Key Responsibilities:

• Provide ongoing support and advocacy for clients to address their physical, emotional, and social needs
• Ensure compliance with all regulatory requirements and standards of care

Qualifications:

• Previous experience working with individuals with developmental disabilities
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team

• Certification in Case Management or related field preferred
