The social worker demonstrates proficiency in providing traditional social work services in a professional and respectful manner with the goal of helping older adults live safely and comfortably in their homes and communities for as long as they can.Salary and Benefits

Salary Range is $70,000- $95,000 Based on Experience

Responsibilities

Monday- Friday Schedule
8:00am- 4:00pm
Mainly Delivering Case Management for the Geriatric Population, Along with Occasional Therapeutic Support.
3-8 Patients per Day, Mostly Outpatient Clinic Based - Both Face to Face and Telehealth
Small Amount of Traveling Required: Local Travel, Within Daily Work Hours, to See a Patient Who is Not Able to Make it Into the Clinic or is in a Facility.
No Nights
No Weekends
No Call

Requirements

Massachusetts LCSW or LICSW License
Minimum of 1 experience in Social Work providing traditional Clinical or Case Management services with a geriatric population
Willingness to Collaborate With Interdisciplinary Team
Collaborate and Provide Communication with Hospitals, Nursing Homes and Community Agencies
Current CPR Certification
COVID Vaccination Required OR Exemption
