As a Client Care Coordinator (CCC), you will be called to care when you’re needed most. As part of Interim HealthCare, you’ll support a full range of patient services to bring comfort and dignity to our clients.

As a Client Care Coordinator (CCC), you will:

• Facilitate new client enrollment with initial and subsequent screening assessments and further maintain client’s healthcare record
• Identify resources to address client’s health, daily living, and safety needs
• Perform administrative tasks such as scheduling, logging, documenting all appointments and services
• Communicate to clients through telephone and computer
• Coordinate with RN and pharmacy medication set-ups and reminders
• Be a family liaison and coordinate with the family, designees, and providers

To qualify to be a Client Care Coordinator (CCC) with us, you will need:

• Education: High School graduate or equivalent
• One year of related business experience or equivalent of education experience
• 1 year of healthcare experience required
• Must have a valid driver’s license
